Social Support Workers

A successful Social Support Worker in our SET4CARE’s team should have:

  • Successful completion or working towards a diploma/degree/post grad in Aged Care/Nursing or related discipline
  • Relevant experience working with Aged Care and/or Disability Care
  • Valid driver’s license
  • Working with Children Check
  • National Police Check
  • NDIS Worker Screening Check
  • First Aid
  • immunisation Records


Our Social Support Workers provide the following services:

  • Personal Care: Provide assistance/standby assistance with daily self-care, such as bathing, toileting, dressing, grooming, getting in and out of bed, moving around the house and/or medication prompting;
  • Domestic Assistance: Provide assistance with domestic chores, including assistance with light cleaning, dishwashing and/or laundry. May also include help with meal preparation where this is not the primary focus of the service;
  • Transport: To assists clients to/from appointments and/or social activities;
  • Social Support: In home or outside the home to access community facilities. This service generally aims to maintain a client’s community connection and fulfil the need for social interaction.

Medical Background Staff

All SET4CARE Medical Background Staffs must have:

  • AHPRA registration or valid nursing licence
  • Clear National Police Check
  • Work Rights Check
  • Full Covid-19 Vaccination
  • Working with Children Check
  • Full Driver’s licence and reliable vehicle
  • Proven nursing experience
  • Excellent knowledge or nursing care methods and procedures
  • Excellent knowledge of emergency care
  • In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.)
  • Genuine interest in working in the community and with the elderly
  • Ability to work autonomously
  • Strong communication, interpersonal and teamwork skills
  • Compassionate and responsible

Our RN/EN responsibilities include:

  • Monitoring patient’s condition and assessing their needs to provide the best possible care and advice
  • Observing and interpreting patient’s symptoms and communicating them to physicians
  • Collaborating with physicians and Case Manager to devise individualised care plans for patients
  • Performing routine procedures (blood pressure measurements, administering injections/medications, monitoring vital signs, writing progress notes/charts/reports etc.)
  • Providing instant medical care in emergencies
  • Fostering a supportive and compassionate environment to care for patients and their families
